Laser Trabeculoplasty
During laser surgery, a strong beam of light is focused on the part of the anterior chamber where the fluid leaves the eye. This results in a series of small changes, which makes it easier for fluid to exit the eye (over time, the effect of laser surgery may wear off, so continued evaluations are needed.) Patients who have this form of surgery may need to keep taking glaucoma medications, but many can reduce or stop using drops.
MicroPulse Laser Trabeculoplasty (MLT)
Micropulse laser trabeculoplasty (MLT) is a new, noncoagulative laser therapy for reducing IOP in eyes with primary open-angle glaucoma and ocular hypertension. This treatment opens the trabecular meshwork by targeting specific pigment cells of the eye and without producing clinically observable heat effects or scarring.
MicroPulse Laser Trabeculoplasty (MLT) can provide the same IOP-lower effects as ALT, or SLT, with less energy and inflammation. MLT improves upon both of the other laser procedures. The eyes remain quiet with minimal postoperative inflammation. Most patients will not require anti-inflammatory drugs after MLT.
The MLT laser is even more comfortable than the SLT, and if need be, can be repeated. MLT employs a series of low power pulses for a short duration, or “micropulse” followed by a pause in laser delivery. The procedure will be essentially the same as SLT, in that most eyes will require two treatments per eye, usually separating the treatment by two weeks.
The MLT uses an advanced laser system to target only specific cells of the eye.

How MLT works
MicroPulse Laser Trabeculoplasty (MLT) is an advancement over other lasers that have been used safely and effectively in the treatment of open-angle glaucoma for more than two decades.
MLT works by using laser light to stimulate the body's own healing response to lower your eye pressure. Using a special wave-length and energy, the laser affects only pigmented (melanin containing) cells of your eye. MLT improves the flow of fluid in the eye, which in turn lowers your eye pressure.
How MLT is performed
MLT is usually performed in the physician's office and only takes a few minutes. Prior to the procedure, eye drops will be given to prepare the eye for treatment. The laser applications are made through a special microscope, similar to the one used for eye examinations.
What to expect after MLT treatment
Your eye pressure may drop as quickly as a day or more after having MLT performed. The doctor may treat the eye with anti-inflammatory eye drops that will be continued after the procedure. Most likely, you will have to return for follow-up visits.

What are the side effects of MLT?
Unlike some glaucoma medications, there are no incidences of allergy or systemic side-effects with MLT. Complications are minimal but may include inflammation, temporary increase in eye pressure, blurred vision, headache, iritis, corneal edema, corneal lesion, conjunctivitis, or eye pain. Will I feel anything with the laser treatment?
Generally, the laser treatment does not cause pain. However, studies show some patients experience a sensation during treatment. You may see a green flash of light, and feel nothing from the laser. The lens used on your eye may cause some redness or irritation.
