Eye Centers of Florida participates with many major insurance plans. Our billing staff will be happy to answer any of your insurance questions at 1.888.393.2455.
Eye Centers of Florida is a Medicare Part B provider. We accept assignments on all Medicare Part B claims. We will submit all Medicare claims for you. By Medicare law, each patient is responsible for the yearly deductible. Patients are also responsible for the 20% co-pay of the Medicare allowable charge. If you have supplemental insurance we will bill your supplement insurance. If there is any remaining balance after Medicare and the supplement insurance payments, the patient is responsible. Medicare does not pay for the refraction involved in your eye exam – this is your responsibility.
Our offices participate in many HMOs and PPOs. As a courtesy to our patients, we will file claims with your insurance company. Please be aware that your insurance reflects a contract between you and your insurance company. You, the patient, are responsible for your bill. It is important that you obtain necessary referrals if your insurance requires them.
For those patients without health insurance you will be expected to pay at the time of service or make payment arrangements with our billing office. Although fees are due at the time of your visit, we will do everything possible to help you structure an affordable payment plan for surgical procedures or long-term treatment.
Please discuss any special financial arrangements with our billing and financial coordinator at the time of, or prior to your appointment. Any amount considered in excess of the “usual” and “reasonable and customary” amount that is not paid by your insurance company becomes your responsibility. You further agree to pay to the practice any balance on your account within 60 days of treatment. Lastly, if any action of law or inequity is brought to enforce this agreement, the practice shall be entitled to reasonable attorney’s fees, court costs, and any other cost of collection required.
Eye Centers of Florida is a provider for many managed care plans. We are on the Vision Plans for Vision Services Plan (VSP), Vision Benefits of America and Aetna Focus. If you are a managed care participant, please provide us with all pertinent information, referral forms and your participation card. Co-pays, co-insurance and/or deductibles will be due at time of service.
Not all services are covered by all insurance health plans. Some services may not be covered by your specific or individual policy. Services that are not covered are the patient’s responsibility.

Potential and active patients should directly contact the other health care providers involved in their care for this information, as it may differ from Eye Centers of Florida.
Patients whose insurance is out of network from Eye Centers of Florida may still be able to get an adjustment on their liability. This is only possible when Eye Centers of Florida is not actively prohibited from offering out-of-network adjustment under state law, Federal law, or the insurance provider. This adjustment will vary based on the insurance plan. Delinquent accounts may lose the adjustment.
